There are two Benefits Statements available, one prepared by the University of Illinois and one by the State of Illinois Department of Central Management Services (CMS). These statements allow you to view your current and past benefit elections, including both mandatory and voluntary University of Illinois and State of Illinois benefit plans.
Note: SURS and RSL BasicCare information is not currently included in these statements.
Your State of Illinois Benefits Information
Information about your State Employee Group Benefits is available on CMS MyBenefits Marketplace.
Click the button above to CMS MyBenefits Marketplace, then use your CMS-issued Employee ID Number (EIN) to log in. This number can be found on a letter from CMS that was mailed to your home address, and is not your University NetID, Enterprise ID, or UIN. If you have issues logging in, or do not have your EIN, use the self-authentication on CMS MyBenefits Marketplace to recover your login information.

Access the University Benefit Statement
Click the Continue button below and login using your University Enterprise ID and password. Log in and select your campus location. Then, click "Employee", "Benefits and Deductions", and finally "Benefit Statement." Follow the on-screen instructions to view your current benefit statement, or enter another date to see a historical summary of benefits.
