Dependent Eligibility Certification Statement and Information

Qualifying Criteria for Dependent Categories

Employees must complete the Dependent Eligibility Certification Statement for dependents on a semi-annual basis; however, supporting documentation is not required for any dependent that remains in the same dependent category.

Note: The cost of continuing coverage for dependent children age 19 and above is the same as that for a dependent child under the age of 19. The only exception to this would be for a Veteran Adult Child that is a Non-IRS tax dependent.

Student
Unmarried child age 19 up to but not including age 24, enrolled as a full-time student in an accredited school. Note: If continuing in the Student category, school documentation is not required.

Sponsored Adult Child
Unmarried adult child age 19 up to but not including age 26. Not eligible for State life insurance.

Student Leave of Absence
Unmarried child age 19 up to but not including age 24, enrolled as a student in an accredited school but is on a medical leave of absence or reduced course load to part-time due to a catastrophic illness or injury. Maximum coverage: 1 year.

Student Military Extension
Unmarried child age 19 up to but not including age 25, enrolled as a full-time student who was a member of the United States Armed Services, including the Illinois National Guard. Eligible for coverage for the amount of time spent on active duty between the ages of 19 and 24.

Disabled
Unmarried child age 19 or older who is continuously disabled from a cause originating prior to age 19 (age 24 if enrolled as a full-time student). In addition, for tax years in which the child is age 27 or above, eligible to be claimed as a dependent for income tax purposes by the employee.

Veteran Adult Child - Non-IRS Dependent
Unmarried adult child age 26 up to but not including age 30, an Illinois resident, has served as a member of the active or reserve components of any of the branches of the U.S. Armed Forces and received a release or discharge other than a dishonorable discharge. Note: Premiums are not tax exempt. Employee must pay 100% of cost for coverage. Not eligible for State life insurance.

Veteran Adult Child - IRS Dependent
Unmarried adult child age 19 up to but not including age 30, an Illinois resident, has served as a member of the active or reserve components of any of the branches of the U.S. Armed Forces and received a release or discharge other than a dishonorable discharge. In addition, for tax years in which the child is age 27 or above, eligible to be claimed as a dependent for income tax purposes by the employee. Not eligible for State life insurance.

Other
Dependent financially dependent on the employee, eligible to be claimed as a dependent for income tax purposes by the employee and continuously enrolled as a dependent in the State of Illinois Insurance Program with no break in coverage since February 11, 1983. Not eligible for State life insurance.                                                            OR
Dependent who received an organ transplant after June 30, 2000, and either eligible to be claimed as a dependent for income tax purposes by the employee or, if a dependent child eligible to be claimed as a dependent for income tax purposes for tax years in which the child is age 27 or above. Not eligible for State life insurance.
