  • Dependents must be enrolled in one of the health insurance plans to be enrolled in dental insurance.
  • All insured dependents must be enrolled in the same health and dental plan as the employee.
  • If both the employee and spouse are insured employees, either or both may elect health coverage for dependents; however, the same dependent cannot be enrolled under both spouses for the same type of coverage.
  • Dependents enrolled in one of the health insurance plans are automatically enrolled in the vision plan, at no additional cost.
  • Dependents enrolled in an HMO health insurance plan must choose their own Primary Care Physician. See the Provider Directories for more information.
  • Employees electing dependent coverage must provide supporting documentation of dependent status (i.e. marriage license, birth certificate, etc.).
  • Social Security Numbers of all dependents are required to be enrolled in the health and dental insurance.
  • If dropping a dependent, the dependent must be dropped from both the health and dental plans.

Note: An Annual Fall Certification is required for dependents in the categories of Disabled, Other and Veteran Adult Child.
