Accidental Death and Dismemberment (AD&D)

Change/Terminate AD&D Coverage

  1. Click the Benefits tab.
  2. Click AD&D in the left nagivation.
  3. Determine if you want to change or terminate your State or University AD&D coverage. You can change University coverage at any time. State AD&D coverage can be updated during the Benefit Choice period, typically in the month of May or when experiencing a qualifying event.
  4. Click the appropriate State Enrollment or University Enrollment link in the left navigation.
  5. Follow the instructions in the online application.

Change Home/Work Address

  1. Click the Personal Info tab.
  2. Click Employee Information form.

Benefit Choice

Change Elections During Benefit Choice Period
Benefit Choice is an annual open enrollment period typically during the month of May, with an effective date of July 1. With very limited exceptions, the Benefit Choice period is the only time changes may be made to Health, Dental, Life, AD&D, FSA, and dependent coverage, unless experiencing a qualifying event.

During Benefit Choice:

  1. Click the Benefits tab.
  2. Click Benefit Choice in the left navigation.
  3. Review your current elections.
  4. Follow the instructions for each online application.

Certification

Add/Change/Delete Certifications

  1. Click the Personal Info tab.
  2. Click Employee Information Form.
  3. Logon using your NetID, Password and PIN.
  4. Click Certifications from the list.
  5. Follow the instructions on the screen for updating the record.

Civil Service Exam

Register for Civil Service Exam
  1. Click the Careers tab.
  2. Click Jobs in the left navigation.
  3. Select the appropriate campus button.
  4. Follow the instructions for Civil Service applications.

Confirm the Status of my Exam Request
To confirm the status of your exam request, you will need to revisit the appropriate campus website, by following the instructions above.

Set Up/Change a Financial Institution for Direct Deposit

  1. Click the Compensation tab.
  2. Click Direct Deposit in the left navigation.

Drivers License

Add/Change/Delete Drivers Licenses
  1. Click the Personal Info tab.
  2. Click Employee Information Form.
  3. Logon using your NetID, Password and PIN.
  4. Select Drivers License from the list.

Earnings Statement

Education

Add/Change/Delete Institution or Degree

  2. Click FSA in the left navigation.
  3. Click the Enrollment link in the left navigation.
  4. Click the Continue button.
  5. Logon using your NetID, Password and PIN.
  6. Follow the instructions to make the appropriate changes or enrollments.

Health Insurance

Life Insurance - State and University Plans

State of Illinois Life Plans

Change/Terminate Optional State Life Insurance

  1. Click the Benefits tab.
  2. Click Life in the left navigation.
  3. Determine if you want to change or terminate your Optional State Life Insurance coverage. This can be done during the Benefit Choice period, typically in the month of May or when experiencing a qualifying event.
  4. To increase your coverage, complete the Statement of Health Form.

Change State Life Insurance
Full-time employees automatically receive term life insurance in an amount equal to their annual salary, at no cost. Part-time employees may elect life insurance but must share in a percentage of the cost proportionate to their appointment.

State Life coverage can be updated during the Benefit Choice period, typically in the month of May or when experiencing a qualifying event.

University of Illinois Life Plans

Change/Terminate Optional University Life Insurance Coverage for Yourself or Dependents

Change Optional University Life Insurance Beneficiary

  1. Click the Benefits tab.

Change University Life Insurance Over $50,000

Retirement

Register for Retirement Planning Seminars
  1. Click the Benefits tab.
  2. Click Retirement - SURS, 403(b), 457 in the left navigation.
  3. Click Orientation Sessions, Seminars, Webinars and Counseling.
  4. Click Retirement Seminars.
  5. Read about the seminars and click Continue to register for any available seminars.

Cancel Enrollment in Retirement Planning Seminars
Enrollment can also be cancelled through the online registration system, by following the instructions above. Or, you can contact University Human Resources at 217-333-2590 or uihr@uillinois.edu.


Service Toward Tenure

Sign Up For or Terminate Shared Benefits

  1. Click the Leave tab.
  2. Click Shared Benefits.

Withdrawal Shared Benefits

  1. Click the Leave tab.
  2. Click Shared Benefits.
  3. Select Withdrawal of Shared Benefits Leave.
  4. Click on Request for Withdrawal Application Form.

Supplemental Retirement Plans - 403(b) and 457

Enroll/Change/Terminate 403(b)

  1. Click the Benefits tab.
  2. Click Retirement - SURS, 403(b), 457 in the left navigation.
  3. Click University of Illinois Supplemental 403(b) Retirement Plan.
  4. Click Enroll in, Make Changes to, OR Terminate my 403(b) Plan.
  5. Follow the instructions to make the appropriate changes or enrollments.
