Birth, Adoption, or Placement for Adoption of a Child

Employees starting or adding to their family may need to make changes to benefits or personal employee information. Please submit the following required documentation to the UPB Benefits Services office on your campus:

  • Copy of the newborn's birth record or birth certificate -OR-
  • Copy of the child's adoption papers stating legal guardianship of the child certificate stamped by the circuit clerk.

Benefits Changes

Changes to the following benefits must be submitted within 60 days of the event.
