Employee is Financially Responsible for a Child's Insurance

An employee receiving a court decree establishing financial responsibility for a child's health and dental insurance can make several changes to benefits and personal employment information. Dependents must meet dependent eligibility guidelines to be eligible for coverage. Employees may initiate changes to the following:

Personal Employee Information

Emergency contact
Tax withholding allowance (W-4)
Direct deposits

Required Documentation

When making a change to benefits or personal information, include a copy of the complete court order or decree.
