Change in Managed Care Plan's Primary Care Provider

When employees in an HMO plan lose their Primary Care Provider (PCP) due to the provider leaving the HMO network, changes can be made to their health plans. Employees may initiate changes to the following:


Health insurance plans

Required Documentation

When making a change to benefits, include a copy of a letter from the doctor or the HMO stating the PCP is no longer accepting new patients or is leaving the network.
