State of Illinois Life Insurance

To change your life insurance coverage, personal information, or beneficiary information, click the button above to CMS MyBenefits. You will need your CMS-issued Employee ID Number (EIN) to log on. This is not the same as your University ID Number (UIN) or NetID. If you have lost or forgotten your EIN, please use the self-authentication process available on CMS MyBenefits.

  • Optional coverage is administered by Minnesota Life Insurance Company.
  • You may apply for insurance for yourself in an amount up to eight times your annual salary with a maximum of $3,000,000 when combined with basic life coverage.
    Note: New Employees may enroll for coverage up to 4 times their salary within 30 days of hire without providing proof of good health. Requests beyond this timeframe or amounts of five to eight times your salary will require a Statement of Health before the coverage request is approved. A Statement of Health is required for any increase in coverage.
  • If elected within 30 days of initial employment or eligibility date, the effective date will be the first day of active State employment or initial eligibility.
  • If elected after the first 30 days of initial employment, the effective date will be the date of the approval by Minnesota Life, subject to the Irrevocability Rule.
  • You may apply for spouse/Civil Union Partner life insurance in the amount of $10,000.
  • You may apply for child life insurance in the amount of $10,000 for each child/child of a Civil Union Partner.
  • Enrollment for employee, spouse/Civil Union Partner, or child/child of your Civil Union Partner life insurance is guaranteed during the first 30 days of employment.
  • After the new employee enrollment period, a Statement of Health is required to increase your life insurance and to add coverage for your spouse. Complete your Statement of Health on CMS MyBenefits by clicking the button at the top of this page.
  • The effective date for newborn children or newly acquired dependents is the date of birth or acquisition, or the date of request for coverage, whichever is later.
  • Employees must designate or change their beneficiaries through CMS MyBenefits.
